MSW Graduation Response Form

School of Social Work Graduation Information Form

  • This is the first step in the graduation process. You will be asked to complete an additional form in the spring semester. In the meantime, please complete this form if you plan on graduating in December 2016, May 2017 or August 2017. Please notify the Office of Student & Academic Services at sswstudentservices@uconn.edu if your graduation date changes after completing this form.

    Please note: If you are completing a 15-hour field placement or taking summer course work you should indicate August 2017 as your graduation date. Grades will not be submitted in time for you to be considered a May 2017 graduate. However, you can still participate in the ceremonies.

    Please submit this form by November 14, 2016.
