Confirmation of Participation Form - MSW Orientation & Advising for Spring 2022 Admits Congratulations on admission into the University, we are lucky to have you joining our institution! According to University policies, satisfying the requirement of Orientation, through attendance and acknowledgement of understanding, are required. Note, this form cannot be completed prior to your Orientation session. To acknowledge your understanding of the content shared, and to verify your attendance, please read and check the statements below. I acknowledge that: Name First Last I.* I listened in on the full duration of the session and understand all the requirements of incoming students. II.* I understand that I must comply with the NASW Code of Ethics and School Policies. III.* I understand my responsibilities and expectations as a Master’s of Social Work student. IV.* I understand that students beginning the program at the start of a Spring semester can complete the program in either 2.5 or 3.5 years. Students completing the program in 2.5 years will have one semester of just coursework (Spring 2022) and then begin their field placement along with co-requisite courses the following Fall 2022 semester. Students completing the program in 3.5 years will have three semesters of just coursework (Spring 2022, Fall 2022, Spring 2023) and then begin their field placement along with co-requisite courses the following Fall 2023 semester. V.* I understand that field placements are completed by committing to 15-20 hours a week at placement. Regular MSW students must complete two (2) separate field placements. VI.* I understand that Field Placements cannot solely take place on nights and weekends, and that a minimum of 8 hours per week must be completed during the organizations normal business hours. VII.* I understand that I am responsible for cost of a background check and health monitoring before a field placement is confirmed.
